DUTIES AND RESPONSIBILITIES: - Ensures the prompt and accurate delivery of medications to patients. - Physician orders and patient medications profiles are reviewed processed and checked. - Drug information is provided to health care professionals and patients to assist in providing rational drug therapy. - Orientation of new staff is carried out. - Appropriate and accurate filling of prescriptions for in-patient and outpatient with detailed instructions by interpreting each prescription before filing to ascertain if strength and dosage prescribed is within patient tolerance. - Reviews patient profile to check for Drug vs. Drug interaction, Drug vs. patient allergies, and Drug vs. patient diet restriction and dosage suitable. - The daily preparations of medicines for in-patients are carried out based on the unit dose system. - Change, verification and modification of in-patient medication for specific patients. - Provide current and accurate drug information to health care professionals. - Confidentiality and security of patient information is maintained as per hospital policy and professional pharmacy standards. - Estimates monthly controlled drug requirements and stock level and control and initiates storehouse orders for all appropriate areas. - Participate in-patient counselling and training of pharmacy student. - Assist in updating of Hospital Formulary. - To ensure accurate and proper storage of any new delivered medicines. - Controls and observes the process of computerization, including dispensing, inventory, and drug vs. drug interaction, allergies, refills and validity of prescriptions. - Reviews the operating manuals for the Pharmacy unit every 2 years. - Participates in continuing education to keep abreast of new pharmaceuticals, equipment techniques and antidote information. - Participates in QI activities/meetings applicable to the Pharmacy. - Keeps a current listing of physician signature specimens, abbreviations and clerical symbols recommending and implementing approved changes. - Implements and maintains infection control procedures; Patient safety advocates. - Complies with hospital dress code. - Perform other applicable tasks and duties assigned within the realm of the employee's knowledge, skills and abilities
